An injury or condition that occurs as a result of a vaccination is termed as vaccine injury or vaccine reactions that have been given to protect against disease. Vaccinations provide very rare serious reactions and are less risky than preventing the disease.

The normal reactions to DTap immunization are mild fever, irritability and localized tenderness and swelling at the site of injection. The abnormal reactions to DTap immunization are unrelieved crying, high-pitched cry, unusual shock-like syndrome (unresponsiveness, hypotonia), and marked increase in sleeping time, persistent high fever and seizure. The normal reactions to MMR are fever up to 103 degrees about 7-10 days after vaccine administration, mild measles rash (reaction to measles vaccine) and transient joint pains (reaction to rubella vaccine). The abnormal reaction to polio vaccine is anaphylaxis or anaphylactic shock. The normal reaction to prevnar (pneumococcal vaccine) is fever.

DTap or DTP/Polio Combination or other vaccine containing whole cell pertussis bacteria, extracted or partial cell bacteria, or specific pertussis antigen(s) causes many symptoms illness, disability, injury, anaphylaxis or anaphylactic shock encephalopathy (or encephalitis), shock-collapse or hypotonic-hypo responsive collapse, residual seizure disorder in accordance with subsection.

The polio vaccines (other than Inactivated Polio Vaccine) causes paralytic polio and also any acute complication or sequela (including death) of an illness, disability, injury, or condition referred to above which illness, disability, injury. Inactivated Polio Vaccine causes symptoms like anaphylaxis or anaphylactic shock.

The measles, mumps, rubella, or any vaccine containing DT, Td, or Tetanus Toxoid will also cause anaphylaxis or anaphylactic shock encephalopathy (or encephalitis)

Abscess (pus) at the injection site is a possible symptom of vaccination. The risk of the disease is far greater than the injury caused by the vaccine considering the severity of the diseases that the immunizations prevent. Vaccine-related incidences are extremely low. Newer vaccines like DTaP and IPV vaccines produce fewer side effects than those used in the past.

The vaccine reactions are tracked and recorded by the Vaccine Adverse Event Reporting System (VAERS) and the National Childhood Vaccine Injury Act.
